Pure Project recently completed the first transitional care facility with Western Health.

The existing Hazeldean extended care facility built in 1970 was converted to a 28 bed transitional care facility to allow Western Health to provided assisted care to patients transitioning back into the community.

Pure Projects was selected following our successful involvement with Western Health and VHBA on the Bacchus Marsh Hospital refurbishment.
